This is the Privacy Policy for the website www.jmlevinemd.com (the “Site”).  This Privacy Policy is posted to provide information about your rights and to explain our practices and is a part of the Terms & Conditions that govern the Site. 

1.  Policy

The policy of Jeffrey M. Levine MD (“DR. LEVINE”) is to respect and protect the privacy of users of the Site.

DR. LEVINE aspires to build users’ trust and confidence by promoting fair information practices.  Accordingly, DR. LEVINE provides this Policy to inform Site visitors of what personally identifiable information (“Personal Information”) is being gathered and for what purpose DR. LEVINE intends to use such Personal Information.  DR. LEVINE will not knowingly exceed the purposes of the use without notification.  Users may give their consent to DR. LEVINE or may choose not to participate in the Site.  DR. LEVINE also identifies the safeguards and measures that it employs to secure the Personal Information it collects from the Site.

DR. LEVINE agrees to exercise reasonable cautions to maintain the confidentiality of Personal Information provided by you in connection with accessing and using the Site.

Because we gather various types of Personal Information from Site users, we feel you should know how we collect, use, disclose, manage, and store the Personal Information that you may voluntarily provide us and the Personal Information that is collected passively through the use of cookies or image tags on our website.  DR. LEVINE agrees to post prominently and in plain language its specific privacy policies.

This Privacy Policy sets forth DR. LEVINE’s current policies and practices with respect to Personal Information of the users of the Site.  Your continued participation on the Site indicates your acceptance of this Privacy Policy and of the collection, use, disclosure, management, and storage of your Personal Information as described below.

2.  Collection and Use of Personal Information

Generally, you may browse the Site without providing any Personal Information.  We do not collect Personal Information about individuals except when specifically and knowingly provided by such individuals.

The Site provides contact information including an email address info@shcny.com that you may use.  If you chose to contact DR. LEVINE, then you will be submitting Personal Information,  e.g., your full name, e-mail address, and perhaps other Personal Information.  You may be added to DR. LEVINE’s mailing list services, so that you would receive information about upcoming lectures, exhibitions and publications.

The Site may passively collect certain information using various technologies and means.  We may receive and record Internet Protocol (“IP”) addresses in the normal course of operation of the Site.  While we may receive and record IP addresses, we will not use them to identify you personally or disclose them to others.  Navigational data related to user visits to the Site may also be recorded for system management and in order to improve the content of the Site.

We may also use your information for operational purposes, or to find out how we can improve the Site via online tracking mechanisms.  We may use aggregate Personal Information collected via cookies to help us evaluate and modify existing features of the Site.  We may also use your Personal Information to develop new online features that may be of interest to you.

We will not sell, rent, or lease your Personal Information to any third party without your consent.However, DR. LEVINE may disclose Personal Information in special cases: (1) when there is reason to believe that disclosing this information is necessary to identify, contact, or bring legal action against someone who may be causing injury to or interference with (either intentionally or unintentionally) the rights of DR. LEVINE or to anyone that could be harmed by such activities; (2) when there is a good faith that the law requires it; (3) to any third party who may acquire the Site; and (4) in situations involving threats to the physical safety of any person.  In addition we may disclose Personal Information to contractors we use to support the Site (such as technical support), in which case we will require such third parties to agree to treat your Personal Information in accordance with this Privacy Policy.  DR. LEVINE does not otherwise distribute to third parties your personally identifiable information.  DR. LEVINE, however, will have no liability for disclosure of information obtained due to errors in transmission or the unauthorized intervention of third parties.

4.  Security of Information

DR. LEVINE has taken reasonable steps to secure the Personal Information collected through the Site in a manner consistent with current industry standards. The importance of security for all Personal Information associated with users of the Site is of utmost concern to us.  Your Personal Information resides on a secure server that only selected persons may access.

Unfortunately, no data transmission over the Internet or any wireless network can be guaranteed to be 100% secure.  As a result, while we strive to protect your Personal Information, you acknowledge that: (a) there are security and privacy limitations of the Internet which are beyond our control; (b) the security, integrity, and privacy of any and all information and data exchanged between you and us through the Site cannot be guaranteed; and (c) any such information and data may be viewed or tampered with in transit by a third party.

LEVINE may use “cookie” technology, but only to obtain non-Personal Information from online visitors in order to improve their online experience and facilitate their visit within the Site.

6.  How You Can Review or Correct Your Personal Information

You may contact DR. LEVINE by e-mail at info@shcny.com to ask if we are keeping Personal Information about you.  If we do have Personal Information about you, we will provide you with a readable copy of such Personal Information at no charge.  Factual errors in your Personal Information, including out-of-date Personal Information, can be corrected by sending us a request that credibly shows our error.

7.  Opt-Out Procedures

You may always opt-out of receiving future mailings or communications from us.  To remove any personally-identifiable information from our database or to not receive future communications from DR. LEVINE, send an e-mail to info@shcny.com with the word “REMOVE” in the subject line.

There may be links provided to other sites and/or resources over which DR. LEVINE has no control.  These links are provided as a convenience to users and should not be construed as an affiliation with or endorsement by DR. LEVINE of any content, items, or services on those third-party sites.  You access and use such other sites, including the content, items, or services on those sites, solely at your own risk.  These linked sites may collect personal information from you when you enter their site.  This collecting of information is not subject to DR. LEVINE’s control.  To ensure protection of your privacy, always review the privacy policies of the sites you visit when linking from this site.  DR. LEVINE makes no representations or warranties with respect to the content, ownership, or legality of any such linked sites.

You acknowledge and agree that DR. LEVINE has no responsibility or liability for the availability of such external sites or resources, or for the content, advertising, products, or other materials available through such sites or resources.

9.  Our Commitment to the Privacy of Minors

DR. LEVINE will not knowingly collect personally identifiable information from site visitors who are under 18 years of age.  We encourage parents to talk to minors about their use of the Internet and the information they disclose online.  If a minor has provided us with personally identifiable information, a parent or guardian of that minor may contact us via e-mail at info@shcny.com if they would like this information deleted from our records.  We will use reasonable efforts to delete the minor’s information from our databases.

10.  Changes to this Privacy Policy

DR. LEVINE may revise this Privacy Policy over time as new features are added to the Site or as Internet law and standards evolve.  We will post these changes prominently.  Changes will not apply retroactively to Personal Information that was collected before the changes to the Privacy Policy, except as may be required by law.
